Annual Vision, Hearing and Scoliosis Screening
The Special Senses and Communication Disorders Act requires that schools show proof that vision and hearing is done on students in grades Primary (4-years-old by September 1, 2018), K, 1, 3, 5, 7 and all new students. This screening is encouraged for grades 2, 4, 6, and 8-12. The school is also mandated by Texas Civil Statutes Article 4477-70 to show proof that spinal screening has been done on females ages 10 and 12 and males age 13. The screening is encouraged for all students 10 years of age and older. You can read more about these requirements here and here.

Because of this law, we have again chosen Tacquard & Associates to perform the screenings on the following dates and campuses:
  • River Campus on Wednesday, February 6, 2019 from 8:30-11:30 AM
  • Creek Campus on Thursday, February 7, 2019 from 8:30-11:30 AM
  • Springs Campus on Friday, February 8, 2019 at 9:00-11:00 AM

These screenings are priced at $30 for one, $45 for two and $55 for three tests.

If your child requires one or multiple screenings, you must fill out this form and submit payment by Thursday, January 24. If you do not submit payment by this deadline, you will be charged for the required screenings, plus a $30 administrative penalty for billing and collection work. If there are any issues with payment, please reply to this message. Next year, we will be absorbing these costs.

If your child had these screenings completed after 5/1/18 and already turned the official results to school, your child does not need to participate in screenings. Please still complete this form and select "I have turned this in."

If your child does not need these screenings, please also complete the form and select, “Not required for my child, I opt out of screenings.”
