Please complete and submit the following forms online as soon as possible. Please reply to this email if you need to discuss further. An FAQ is located here
  1. Physicians’ Form - Print and have filled out by your Physician
  2. Immunization Records: New families must provide immunizations records for all new students in accordance with the State of Texas Requirements for Grades K-5 and 7 (see here).For families with waivers, please be sure a valid waiver is on file with the school. We must have a notarized original form. This can take up to two weeks to receive via the mail.  Click here to request.
Other Important Information
Throughout the summer, we sent various communications to help families get set up for the school year; here are those emails in order so you have the important information you need:
  • For supplies, your child will need a labeled backpack, reusable water bottle, and a lunchbox. We supply your child with all the other classroom supplies they will need while at school. If additional supplies are needed for projects, your guides will let you know.
  • Headwaters 2019-2020 Student & Family Handbook: After you read the handbook, take a moment to login to your Nest Account and fill out one form per child attending Headwaters for the 2019-2020 school year. This form also includes permissions for other important things like High School off-campus, field trips, photo permissions, and more. Only one parent per child is required to sign the form as well as the student. Users will ONLY be able to sign for themselves and the form will not be complete until one parent and student complete the form.
Change In Authorized Pick-Up Permissions is used when you need to long-term add one or multiple people to your list of approved pick-up people. The school will call to confirm this with you before adding. If this is a one-off, please email the attendance office at creekcampusattendance@headwaters.org.
