Health Screenings - River
River Campus
Headwaters School will be conducting vision, hearing, and spinal screenings for required students on each campus in mid-November. All required screenings are covered by your tuition and will be performed by Tacquard & Associates.
Results will be sent home when available. The school will also receive a copy to keep on file.
Students Receiving Required Screenings:
River | Vision & Hearing Screening: Grade 7 and all new students Spinal Screening: Grade 7 girls and Grade 8 boys | Tuesday, November 19, 2019 8:30-11:30 a.m. |
Creek | Vision & Hearing Screening: K, 1, 3, 5, and all new students Spinal Screening: Grade 5 | Wednesday, November 20, 2019 8:30-11:30 a.m. |
Springs | Vision & Hearing Screening: Primary (4-years-old by September 1, 2019) and all new students | Thursday, November 21, 20199:00-11:00 a.m. |
**IMPORTANT: IF YOUR CHILD WEARS GLASSES OR CONTACTS PLEASE MAKE SURE THEY BRING THEM TO THE SCREENING | ||
The Special Senses and Communication Disorders Act requires that schools show proof of vision and hearing screenings. Texas Civil Statutes Article 4477-70 requires schools to show proof of spinal screenings.
OPTIONAL: These screenings are recommended for all students. If your child is not in one of the required grades and you wish for them to have screenings performed, sign up and submit payment via this form by Friday, November 15. Optional screenings are priced at $30 for one, $45 for two and $60 for three tests.
