Please complete and submit the following forms online as soon as possible. Please reply to this email if you need to discuss further. An FAQ is located here
  1. Physicians’ Form - Print and have filled out by your Physician
  2. Immunization Records: New families must submit records in accordance with the State of Texas Requirements for YCC and Primary (see here). For families with waivers, please be sure a valid waiver is on file with the school. We must have a notarized original form. This can take up to two weeks to receive via the mail.  Click here to request
 
Other Important Information
Throughout the summer, we sent various communications to help families get set up for the school year; here are those emails in order so you have the important information you need:
  • For supplies, your child will need 
  • Headwaters 2019-2020 Student & Family Handbook: Please read this handbook. On your first day, please stop by the Front Desk of the Manchaca Building and Bridget Brown, the Campus Manager, will have an acknowledgement form to sign-off on. 
  • Discipline and Guidance Acknowledgement: Please read this document. On your first day, please stop by the Front Desk of the Manchaca Building and Bridget Brown, the Campus Manager, will have an acknowledgement form to sign-off on. No need to print this one.
